DOT Physicals
DOT physical is an FMCSA medical examination for commercial motor vehicle (CMV) drivers to determine if a driver is physically, mentally, and emotionally fit to operate their vehicles. Here, the DOT stands for Department of Transportation.
FMCSA requires all CMV drivers to complete and pass the Department of Transportation physical to ensure public and driver safety. Passing this examination will validate your commercial driver’s license (CDL) for 24 months.

What Awaits You in a DOT Physical?
During your DOT physical exam, a certified CME will check your medical history and vital signs and perform some necessary hands-on physical examinations on you:
- General Appearance
- Eyes
- Ears
- Heart
- Lungs and chest
- Extremities
- Mouth and throat
- Nervous system
- Back and spine
- Hernias etc.
If you can meet the DOT physical requirements, the CME will issue you a DOT medical card or medical examiner’s certificate (MEC). You need this DOT medical card to keep your commercial driver’s license. To get a DOT physical exam, you must complete and submit forms at least 45 days before the examination.
When Will You Need a DOT Physical?
A DOT physical is required in the following circumstances:
- Drivers operating a motor vehicle for interstate commerce with a gross combination weight rating.
- Operating CMV with a gross combination weight of over 10,000 pounds
- Operating CMV with gross vehicle weight rating
- Operating a motor vehicle that can carry more than 15 individuals.
- Drivers are getting paid to operate motor vehicles that can carry more than 8 individuals.
- Operating vehicles to transport hazardous materials that require your vehicle to be placarded.
