Co-Pays vary between insurance plans and individual benefits. The majority of patients co-pay’s range between $20 to $50 but we have seen some plans with a $5 co-pay and others with a $150 co-pay. If you want to know your co-pay before your visit, please call your insurance company with your insurance details and they will be able to provide you with you benefit information. The member service phone number is displayed on the back of your insurance card. If you have deductible, you may owe more, and we will bill you for the balance.

It varies between insurance plans and individual benefits. At the time of service, we collect the co-pay (if you have one), then we bill your insurance company for the services that we have provided. For in-network services, if you have already met your deductible or don’t have a deductible/co-insurance then insurance company will pay based on their contracted rate.

If we are out of network with your insurance or you have a deductible that has not yet been met or you have a co-insurance, then we will bill you for the balance based on the insurance companies contracted rate. Our self-pay initial visit rate to be evaluated by a provider is $105.

Our initial visit cost with the physician is $105. X-Rays, procedures, lab services, etc. are additional. Visit our Self-Pay page for more information or call our office for specific pricing for your needs.

If you are experiencing life threatening conditions such as significant chest pain, shortness of breath, rectal bleeding, or passing out (syncope), extremely high fevers in the young or the elderly, these types of issues are best treated in an Emergency Room at your local hospital. Although our offices are staffed by board certified physicians, life threatening emergencies often require advanced levels of care available only at a hospital.
